Our Community Outreach service aims to achieve person centred outcomes through support in the community.

Who is Community Outreach for?
Community Outreach is for anyone who feels they need support and help with their day to day lives.

How can we support you?
We offer support based on people’s needs and aspirations. This can include support with:

  • household tasks
  • personal care
  • maintaining a tenancy
  • accessing education
  • accessing employment
  • travel and independence training
  • taking medication
  • money management
  • building links with friends, family and the community
  • social and leisure activities
  • making healthy lifestyle choices.


When is Community Outreach available?
Our support is based on individual need and our support is available from just a couple of hours a week to 24 hours a day.

Why Community Outreach works?
Community Outreach enables individuals to access their community through support in acquiring new skills so that they are able to live more independently.

Where is Community Outreach offered?
We offer Community Outreach at an individual’s home or in the community throughout West Wales.

How can you access Community Outreach?
You can access Community Outreach using your personal budget or other form of direct payment, by paying with private funds, or with funding directly from your local authority.
